This highly acclaimed two-day CTG Masterclass course is beneficial for all midwives, obstetricians and clinical negligence lawyers who are involved in interpreting CTG traces. A basic knowledge of CTG interpretation would be beneficial, but not essential.

There has been a well-documented need for multi-professional improvements to CTG interpretation for decades to improve neonatal and maternal outcomes.

Errors with fetal heart rate monitoring was the most common theme found in Five years of cerebral palsy claims: A thematic review of NHS Resolution data (2017).

NHS England (NHS England, 2019, 2016) recommends effective fetal monitoring in labour as one of the key elements of care designed to tackle stillbirth and neonatal deaths, and that training in CTG interpretation should be annual.

AIMS OF THE COURSE

A two-day version of our popular CTG Masterclass, which looks in more depth at CTG monitoring practices. It aims to enable midwives and obstetricians to be the champions of change in their local maternity units; to bring about a paradigm shift in CTG interpretation based on fetal physiology, so as to reduce unnecessary operative interventions whilst improving perinatal outcomes. The course will be led by experts in the field including Mr. Edwin Chandraharan and his research team from St George’s Hospital, London.

COURSE CONTENT

DAY ONE

• Continuous CTG: scientific evidence and the bitter truths.
• Individualising care: considering the wider picture and understanding the pitfalls of our current “Guidelines”.
• Pathophysiology of CTG: understanding fetal heart rate control and physiology behind ‘CTG patterns’.
• Unusual fetal heart rate patterns: sinusoidal, saltatory and overshoots.
• Understanding placental oxygenation: the fetal ‘oxygen cylinder’.
• Recognising pre-existing injury: chronic hypoxia and non-hypoxic antenatal insults.
• Recognising CTG features of different types of intrapartum hypoxia and their management.
• Interactive review of cases.

DAY TWO

• Re-cap: understanding the types of intrapartum hypoxia in interpreting CTG.
• CTG monitoring in high risk labour: meconium, infection etc.
• Fetal acid-base balance and the role of fetal blood sampling.
• Can we determine oxygenation of central organs? Role of fetal ECG.
• Principles of intelligent (intermittent) auscultation and the rational use of clinical guidelines on CTG.
• Mechanism of fetal neurological injury in different types of hypoxia.
• Illustrative cases: interactive discussion.
• Translation of learning into practice: from the horse’s mouth.
