The Need For Training

“Several previous reports have highlighted that critical care is a management modality not a place and should be provided wherever the woman is located and should not be delayed by bed availability on the critical care unit.” (MBRRACE-UK, 2017)

Women accessing maternity services in the UK have more complex health needs than ever before; the majority of women who die during or soon after pregnancy have existing co-morbidities, such as cardiac disease, or other identified vulnerabilities. The latest MBRRACE-UK confidential enquiry into maternal mortality and morbidity (2018) showed a slight increase in the rate of maternal deaths in the UK, with rates now at the same levels as those in 2010-12. Action must be taken if we are to achieve the national Maternity Ambition to halve the rates of maternal deaths by 2025 (DoH 2017).

Healthcare professionals within the maternity team need to be equipped to identify and care for the increasingly complex needs of women; when these women become unwell, they should have immediate access to enhanced or critical care which is dictated by clinical need, rather than location.

AIMS OF THE COURSE

This exciting and pertinent one-day, multidisciplinary course will teach the physiology, early recognition and management of the acutely unwell mother.

• To be able to identify and appropriately escalate the deteriorating patient.
• To be aware of the standards required from staff managing the unwell mother with acute disease in pregnancy.
• To be aware of the effect of ‘human factors’ on the safe and effective obstetric care of a deteriorating patient.
• To be able to interpret information/results (ABG, ECG) relevant to the unwell patient.
• To be aware of the safe and appropriate use of invasive monitoring in the context of a high-dependency patient.

COURSE CONTENT

The course will include lectures, workstations, and group discussions in the following key areas:

• Cardiovascular and respiratory physiology in pregnancy; effective monitoring and management of cardio-respiratory compromise.
• Renal physiology in pregnancy and the importance of urine output.
• Identification and management of maternal sepsis.
• Neurological assessment and management of the obstetric patient.
• Developing an appreciation of the role of ‘human factors’ in delivering safe and effective care.
• Workstations, including; ECG and ABG interpretation, invasive monitoring, and case-based discussions.
