The course will be particularly relevant to the maternity setting, but the principles, techniques and methodology taught is equally applicable to all types of healthcare incident investigation in accordance with the national Serious Incident Framework
About the course
The course is based around a week-long healthcare incident simulation and utilises an array of practical exercises throughout to enable consolidation of learnt skills.
These exercises range from delegates conducting interviews with healthcare professionals and supporting actors, to interpretation and analysis of evidence, developing findings, and writing recommendations. Practical exercises are based on a simulated event that is based on real-world scenarios, so delegates will gain experience conducting a high-fidelity simulated investigation.
Course Structure
This course uses a combination of online self-study and live online training. Delegates complete two days’ worth of self-guided study at their own pace using Cranfield University’s virtual learning platform, before attending three full days of online live training. All of the self-guided study must be completed before the live training takes place, as skills will be practised and built on throughout the course. This format allows for maximum flexibility whilst ensuring that the training is impactful and relevant to all participants. Delegates may access IT support via Cranfield University.
Course Content
Specialist training will be provided in the following areas:

- Gathering and recording evidence.
- Establishing and recording a sequence of events.
- Identifying and recording causal and contributory factors relating to the case.
- Engaging and involving families.
- Interviewing techniques (structured, semi-structured, cognitive).
- Developing and writing appropriate reports and healthcare recommendations.
- Delivering and understanding of regulatory and NHS patient safety policies.
- Understanding systemic investigations and human performance issues.
- Understanding organisational factors and safety culture.
The Need for Training
The way in which healthcare organisations respond and learn following patient safety incidents has repeatedly been highlighted as a cause for concern.
- The Royal College of Obstetricians and Gynaecologists’ Each Baby Counts reports(2018, 2020) highlight significant variability in local maternity investigations, the lack of family involvement, and the tendency to blame individuals rather than fully seeking to understand and address systemic issues.
- The Better Birthsreport (2016) called for a ‘national standardised investigation process when things go wrong’ and highlighted the lack of national training as a concern.
- In 2020, the Care Quality Commission (CQC) published a briefing documentabout progress maternity safety that again highlighted ‘…a lack of investigation and learning when things go wrong’ as an issue.

Baby Lifeline and Baby Lifeline Training have partnered with the world-renowned Safety and Accident Investigation Centre at Cranfield University to a develop a practical, hands-on training course specifically in healthcare incident investigation.
